Every decision has a desired outcome (1st order) consequence and each consequence has a subsequent consequence. One single decision can have chain effect on causal chain effect.
For example, in order to solve rat problem, the government announced bounty for rat tails. Then people started breeding rats for bounty, thereby making the situation worse.
